New patients may schedule an appointment by contacting our office directly by phone or through our online consultation request form. Our staff will work with you to find an appointment time that accommodates your schedule.

Green Life Hematology and Oncology accepts referrals from primary care physicians, internal medicine specialists, hospitalists, and other specialist physicians throughout Tennessee and the surrounding region.

Physicians wishing to refer a patient may contact our office directly. We strive to accommodate urgent referrals promptly and will communicate with the referring provider throughout the course of care.

Preparing Medically

To make the most of your consultation with Dr. Weiss, consider preparing a written summary of your medical history, symptoms, and any questions you wish to discuss. The more thoroughly Dr. Weiss can understand your history, the more precisely he can evaluate your condition.

Please arrive 15–20 minutes before your scheduled appointment to allow time for check-in, verification of insurance information, and completion of any remaining intake paperwork.

Our practice is located within The Mall at Green Hills at 2126 Abbott Martin Rd, Nashville, TN 37215. Ample parking is available within the mall facility.

The Consultation

Depending on your clinical presentation, Dr. Weiss may order diagnostic laboratory tests, imaging studies, or additional specialty evaluations. Orders will be placed during your visit, and our staff will assist you in coordinating follow-up testing as needed.
